扶严宁联合富马酸卢帕他定治疗中度急性手部湿疹的临床研究

Clinical study of triamcinolone acetonide acetate and econazole nitrate cream plus lupatadine fumarate in the treatment of moderate acute hand eczema

：目的 探讨扶严宁联合富马酸卢帕他定治疗中度急性手部湿疹（HE）的应用价值。方法 110 例中度急性 HE患者随机分为对照组和观察组，对照组患者给予扶严宁治疗，观察组患者给予扶严宁联合富马酸卢帕他定治疗，治疗 14 d。采用湿疹面积及严重程度评分、视觉模拟评分法（VAS）与皮肤病生活质量指数（DLQI）评分表评价比较 2 组患者 的疗效、瘙痒程度和生活质量。酶联免疫吸附法检测并对比两组的Th1 细胞[白介素-2（IL-2）、肿瘤坏死因子-α（TNF-α）] 和Th2 细胞[白介素-4（IL-4）、白介素-5（IL-5）]的水平。结果 与对照组相比，观察组治疗 14 d后的IL-2 和TNF-α质 量浓度明显升高，IL-4 和IL-5 的质量浓度则明显降低（P<0.01 或 0.05）。观察组治疗 14 d后的VAS评分和DLQI评分均 明显低于对照组（P<0.01）。结论 采用扶严宁联合富马酸卢帕他定治疗中度急性HE可纠正Th1/Th2 细胞亚群分化失 衡，减轻瘙痒程度和提高患者生活质量。

Abstract:

Abstract: Objective To investigate the triamcinolone acetonide acetate and econazole nitrate cream plus lupatadine Fumarate in the treatment of moderate acute hand eczema (HE). Methods A total of 110 patients with moderate acute HE were randomly treated with triamcinolone acetonide acetate and econazole nitrate cream (control group) or triamcinolone acetonide acetate and econazole nitrate cream plus lupatadine fumarate (observation group) for 14 days. Curative effect, degree of itching, and quality of life were determined by Eczema area and severity index, Visual Analogue Score (VAS), dermatology life quality index (DLQI) scoring table. The levels of Th1 cells (IL-2, TNF-α) and Th2 cells (IL-4, IL-5) in two groups were detected by enzyme linked immunosorbent assay, respectively. Results Compared with the control group, the observation group showed significantly higher concentrations of IL-2 and TNF-α after 14 days of treatment, while the concentrations of IL-4 and IL-5 were significantly lower (P<0.01 or 0.05). The VAS score and DLQI score of the observation group after 14 days of treatment were significantly lower than those of the control group (P<0.01). Conclusion The treatment of moderate acute HE with triamcinolone acetonide acetate and econazole nitrate cream plus lupatadine fumarate can correct the imbalance of Th1/ Th2 cell subpopulation differentiation, reduce itching severity, and improve patient quality of life.
